Description
Stoke-on-Trent City Council is seeking to appoint one Provider for Lot 1 and one Provider for Lot 2 to deliver two specialist short-term home-based support services for adults aged 18 and over.
The services are designed to support people to remain safely within their own homes during periods of increased need, crisis, recovery, assessment or transition, whilst avoiding unnecessary hospital admissions and long-term residential care placements.
Providers may bid for one or both Lots; however, each Lot will be awarded to a single Provider.
Lot 1 - Night Support Service
Lot 1 is a short-term overnight support service delivered through block rota arrangements. The service is intended for individuals who require short-term support, supervision, observation or reassurance during night-time hours from 11pm - 6am.
The successful Provider will deliver overnight support within an individual's home, typically during periods of crisis, hospital discharge, deterioration in health, carer breakdown, increased risk or whilst assessments are being undertaken. The service will provide a physical staff presence overnight to monitor wellbeing, manage identified risks and respond to support needs as they arise.
A key function of the service is to gather evidence about an individual's overnight needs and provide professional observations to support future care planning and decision-making by the Council.
Lot 2 - Wrap Around Support Service
Lot 2 is a flexible short-term wrap around support service delivered through block support arrangements for individuals who require a higher level of support throughout the day.
The successful Provider will deliver short-term intensive home-based support to individuals whose needs require regular monitoring, observation and intervention across extended periods of the day. The service is designed to bridge gaps in support, promote recovery and independence, and provide evidence to inform longer-term care and support planning.
