Description
Manchester City Council requires a provider to deliver a city-wide Early Help for Adults Navigator Service for adults experiencing multiple disadvantage. The service will provide intensive, strengths-based and trauma-informed support to individuals facing complex and interconnected challenges, including housing instability, mental health needs, substance misuse, domestic abuse, social care needs and contact with the criminal justice system.
Through dedicated Navigator workers, the service will help individuals access appropriate services, build resilience, improve independence and achieve sustainable outcomes. The provider will work collaboratively with Adult Social Care, health partners, housing organisations, voluntary and community sector partners and people with lived experience to deliver a coordinated, person-centred and preventative approach that supports wider system change and reduces inequalities across Manchester.
