Tender for the Provision of Extra Care Services

LBH is seeking to commission a care and support Service for older adults (mainly aged 55+), with a range of needs, who reside in the Bridgewater House and Park Lodge Schemes and (age 18+), with a range of needs, who reside in the Bristol Court Extra Care Scheme. The purpose of the Service is to provide high quality care and support which enables and encourages Residents to live as independently as possible within the Schemes for as long as possible.

The Service must be able to meet a diverse range of needs, including people with a physical disability, frailty, dementia, learning disability, autism and mental health conditions.
